What Managed Long Term Services and Supports will be available from Care1st Health Plan?
Long-term care services and supports help you with ongoing personal care needs. If the member meets eligibility criteria, the Medi-Cal services below will be covered by Care1st Health Plan.
If a member does not currently get these services now, Care1st Health Plan can help you get them in the future, if they are medically needed.
- In-Home Supportive Services (IHSS)
In-Home Supportive Services (IHSS) are personal care services for people who need help to live safely in their homes. If a member has IHSS, he or she does not have to change IHSS providers and the member can still hire, fire, and manage providers. The county IHSS social worker will still assess the member's needs and approve the IHSS hours. Member's rights to appeal will stay the same. Care1st Health Plan can work with the member and his or her preferred IHSS providers to make sure the member gets the care needed.

- IHSS Health Care Certification
To initiate an IHSS Assessment, a licensed health care professional must sign a Health Care Certification form (SOC 873) indicating that the member needs assistance to stay living at home. The form must be submitted to IHSS before an IHSS Social Worker can go to the beneficiaries home to conduct an IHSS assessment.
- Community-Based Adult Services (CBAS)
Community-Based Adult Services (CBAS) is daytime health care at centers that provide nursing, therapy, activities and meals for people with certain chronic health conditions. Care1st Health Plan can work with the member and the member's doctor if this service is needed. Please see Community Based Adult Services (CBAS) for additional information.
- Multipurpose Senior Services Program (MSSP)
Multipurpose Senior Services Program (MSSP) provides social and health care coordination services at home for people age 65 and older. Care1st Health Plan can work with the local MSSP provider to better coordinate the member's care.
- Long Term Care in a Nursing Facility
Long Term Care in a Nursing Facility: If the member receives long term care in a nursing home, Care1st Health Plan can work with the member's doctor and nursing home to give the member the same services currently being received and better coordinate the member's care.
